“Helping people find what makes them feel great”

Embedded in GP surgeries I am offering non – clinical support for people experiencing health, social, emotional or practical problems. So they can be empowered to explore what matters to them, to improve their health and wellbeing.

How I’ll work:-

Through person –centered conversations to reflect on areas of their life and wellbeing, to encourage positive behavior change within themselves. Using psychosocial interventions including, but not exclusively, reflective questioning, motivational interviewing and solution focused planning.

I will manage and prioritise my own caseload in accordance with the health and wellbeing needs of your practice. Where required I’ll discuss and/or refer people back to other health professionals in the PCN. I will also connect people to community groups and agencies for practical and emotional support.

Who I can work with:-

I can work with anyone 18 years and over (possible 16 plus, to be discussed) with no upper age limit, Aimed, but not exclusively, for those with complex needs and long term health conditions (e.g. Depression, Stress, Anxiety, High Intensity patients).

How to refer people:-

I can accept referrals from all practice staff in GP surgeries; Nurses/ Reception Team/ GP’s/ Community Health Navigators etc. If someone in the practice thinks a patient would find Social Prescribing useful then refer them to me.

First I’d speak to the person and arrange an initial appointment to introduce my role and find out about them. I’d explore with them if the service is best suited for them and book further appointments if needed.

I’m starting by accepting referrals verbally (come and see me when I’m in your surgery) AND via ‘Task’s on EMIS and System 1. This is a new service and I’m eager to shape it to the needs of your practice and patients!
