District nurses play a crucial role in the primary health care team. They provide high quality nursing care for housebound patients and support for carers and families.

They will assess the health care needs of patients and families and provide care to individuals with short term and long terms needs, chronic conditions and palliative care.

District nurses also play a vital role in reducing hospital admissions, preventing re-admissions and early discharge from hospital allowing patients to return their own homes.

As well as providing direct patient care district nurses also have a teaching role, working with patients to promote independence.
